Services We Provide

  • Well-child exams

  • Sick visits

  • Minory injury treatment

  • Vision, dental, and other health screenings

  • Immunizations

  • Classroom presentations as requested

  • Prevention and wellness services

  • Alcohol and drug screenings counseling

  • Mental and behavioral health counseling

  • Reproductive health services (we do NOT prescribe or dispense birth control)

Services provided for:

  • Anyone between the ages of 0-18 (19 if enrolled in school)

  • Any child, even if we are not their primary care provider

  • Any child – regardless of the ability to pay, insurance status, immigration status, etc.

Access:

  • Walk-in or make an appointment

  • Anyone aget 14 and over can consent for their own treatment for mental and behavioral health services

  • Anyone age 15 and over can consent for their own medical care

  • Anyone meeting the above age requirements can request their visit be confidential, which means the patient gets to decide who gets access to their health information

Payment:

  • We offer a Sliding Scale Fee, which often results in zero cost to patients and families

  • We connect patients to insurance when possible

  • We have access to patient funds, which may assist with transportation or costs associated with referrals for additional services outside of Mosaic
